CFHC Community Partners To ensure a smooth and seamless delivery of services, CFHC continues to develop crucial partnerships with a wide range of organizations. Such participation in or progress toward collaborative linkages include:
Community Health Worker Program: As members of the community, Community Health Workers are highly effective guides to better health in our communities. Community Health Workers provide a unique service within the health care system. CHWs are bilingual, bicultural individuals who serve as a bridge between the health care system and patients. They bring a variety of health education topics to the community. Community Health Workers are effective and help improve health outcomes among minority populations. Unlike many other health care professions, however, there is no standardized training and credentialing program. CHWs attend “train the trainer” programs and commit to share their knowledge in their communities.
Integrated Service Networks: The Community Health Centers Alliance, the Heartland Rural Health Network, Hardee and Highlands Health Task Forces, and the Polk Health Care Alliance (HCAP).
Other Health and Human Services: Florida’s Department of Children and Families, Healthy Start Coalition of Polk, Hardee and Highlands Counties, Healthy Kids, Health Departments, United Way of Central Florida, Redlands Christian Migrant Association, East Coast Migrant Head Start Project, and Mid-Florida Mental Health Services.
State/Local/National Associations: The Florida Association of Community Health Centers (FACHC), the National Associations of Community Health Centers (NACHC), Florida Rural Health Association, National Rural Health Association (NRHA), and Farm Workers Health.
Tri-County Connections (Polk, Hardee, Highlands): Homeless Coalitions, Polk Housing and Neighborhood Development, Polk Health Care Plan, Polk Vision and numerous local groups.

Health Education Training: Central Florida Area Health Education Centers. CFHC is a “teaching” health care organization, rotating medical students from NOVA Southeastern University. An agreement to rotate nursing students is in place with South Florida Community College.
